TDCJ Forms

These forms are used throughout the year. 

• Special Volunteer Requests: No initials are needed in the form.

• Bringing Food: Submit the Food Donation and the Materials Form together.

• PDF files: Can be printed out, filled in by hand, and sent to Chaplain.

• Approval Time: Forms typically take 2–4 weeks to be processed, depending on the facility.
