TDCJ Forms

These are common Texas Department Criminal Justice (TDCJ) forms used by Volunteer Leaders throughout the year.

New inmates joining the program mid-year should complete the Bio form below. 

Food brought into the unit needs both the Food Donation and Materials Donations forms completed. Approvals take 3 weeks.

Note: If buying food from Costco use FL# 1146 for food license number.

Sometimes Chaplains will use this form to fast track food and book approvals.

Use this form to get camera approvals. Include the type of camera and lenses you will bring in. You cannot film with an iPhone. To fast track approvals limit media sharing to non-public sites and for use in local churches.

The volunteer leader or program coordinator will place all returning-approved volunteers on this Approved list and send to the Chaplains in January to be put them on the gate list for February.
